Purpose of agreement

The purpose of this agreement is to document a personalised and self-directed support arrangement between (participant’s name) and Registered Music Therapist, Tara Del Borrello.

Tara Del Borrello who provides the service user the flexibility and authority to determine his/her chosen supports to achieve his/her potential /aspirations and control his/her own life. (Music Therapist) Tara Del Borrello agrees to provide the services outlined in this Individual Service Agreement. Any changes to the services listed in this agreement will require prior authorisation from all parties.

Client rights and responsibilities

Whilst accessing services outlined in this Agreement as a client of Sol Music Therapy

• Have the right to be treated with dignity and respect and to have my choices and aspirations supported as far as is reasonably possible

• Have the right to determine the type and range of activities that I wish to participate in

• Have the right to privacy and confidentiality

• As a family member I, (parent/guardian name) will:

• Keep the Service provider informed of any changes in personal life such as where I live and any changes in (participant’s name) medication (if applicable).

• Work cooperatively with (Music Therapist) Tara Del Borrello regarding issues arising during the development and delivery of support and activities covered by this agreement

• Pay all fees owing by the due date. All fees must be paid within 14 days of issued invoice. Late fees will apply if fees are not paid by the due date, late fees will accumulate $20 flat fee on top of the fee owing after the due date of payment.

• Sol Music Therapy will do their best to inform and remind you to pay before due date, however, please keep in mind it is yours and/or planners responsibility to ensure fees are paid within 14 days, unless due to unforeseen circumstances which you must discuss with management, the possibility to dismiss the accrual of late fees.

• Adhere to the budgetary requirements of my service plan

• Provide Sol Music Therapy with 2-weeks advance notice of intention to leave the service.

• In agreeing to provide this support arrangement (Music Therapist) Tara Del Borrello:

• Will respect the rights of the client to determine the range and types of activities they wish to participate in.

• Will work cooperatively and in line with the principle of least restrictive alternative with the client and the activities they have chosen to undertake.

• Will treat information about the client and their activities as private and confidential in line with the client’s wishes and with privacy legislation.

• Will be responsible for ongoing liaison with the relevant funding body regarding the development and operation of the support arrangement.

• Will respect the right of the client to determine the range and type of activities they wish to participate in.

• Will advise the client of any developments that may affect the way support is provided.

• Will provide the client with 2 weeks notice of intention to cease service provision.

Complaints and Disputes

Sol Music Therapy recognises that clients and their carers have a right to provide feedback to Tara Del Borrello, staff or management to raise suggestions, resolve grievances and commend good performance and encourages all clients to speak up when they are not happy.

Service Fees

Variations may be negotiated depending on the client’s choice and needs and availability of service/supports required.

Review Report

Tara Del Borrello will provide review reports within 1 month of the request. Carers may request updates during this period.

Cancellation Policy

Sol Music Therapy will need to be informed 24 hours in advance if (participant’s

name) is unable to make their appointment without charge. If less than 24-hours’ notice is given, the appointment will need to be paid for in full amount.

Terms and Conditions for Bonding With Bubs and Rhythm Rascals

By enrolling in Bonding With Bubs or Rhythm Rascals, you agree to the following terms and conditions:

Commitment:
You are committing to a full term, which typically runs for 8-10 weeks. If a group has fewer than four sign ups, Sol Music Therapy reserves the right to cancel the session time. In this case, you will be notified before the term begins and offered alternative session times to join. If the alternative times do not suit, you will have the option to defer your enrollment to the next term or receive a refund.

Session Fees:
Fees vary depending on the length of the term.

Session Usage:
All sessions must be completed within the current term. Missed sessions are non-refundable and cannot be transferred to future terms or to another person.

Payment:
Full payment for the term is required before the first session. Failure to pay may result in the forfeiture of your place in the program.

Refund Policy:
Refunds will only be issued if Sol Music Therapy cancels a session and no suitable alternative time is available. Refunds are not available for missed sessions, changes in personal plans, holidays, or other reasons.

Facilitator Absence:
In the event that the facilitator is unable to attend a session due to illness or unforeseen circumstances, a replacement will be arranged where possible. If a replacement is unavailable, a makeup session will be scheduled within the term or during the first week of the school holidays.

Term Booking:
By booking a term, you secure your place for the entire duration. Group sizes are limited, and we cannot accommodate changes or transfers once the term has started.

Family Discount:
Family discounts are available for families enrolling two or more siblings. Please contact us for more information.
